If your cultures keep coming back negative and the antibiotics never fully help, the working diagnosis may simply be wrong. Interstitial cystitis / bladder pain syndrome, sexually transmitted infections, vaginitis, and urinary stones all present with burning, urgency and frequency — and none of them is cured by another course of antibiotics.

A patient on their fourth antibiotic course for "recurrent UTI" with four negative cultures is not a treatment-resistant UTI. They are an unmade diagnosis.
