You have burning, urgency, and a course of antibiotics that did not work. Or a culture that came back "no growth" while you clearly have something. Both are common, and both have the same root cause: urine culture takes 24–72 hours and it does not find everything. Fastidious organisms, polymicrobial infections — culture can miss them entirely.

We run a molecular PCR panel instead. It identifies the organism directly from its nucleic acid, and it detects antibiotic-resistance genes at the same time. What you are looking at: deep blue is the nucleus of a bladder urothelial cell; the magenta web is the membrane and cytoskeleton. The green rods clustered along the cell edge are uropathogenic E. coli completing the first step of infection — adhesion. It matters more than it sounds: a bacterium that cannot grip the urothelium usually gets flushed out. Some UPEC go further and invade the cell, forming intracellular bacterial communities — a hidden reservoir, and one reason infections come back after treatment "worked."

One honest caveat about resistance genes

A resistance gene is a fast risk signal — it is not a full susceptibility panel. Molecular testing tells ythe partner-network physician what the organism is and what it is likely to resist, quickly. Culture and susceptibility testing directly measure how the organism responds to specific drugs. If your UTIs keep coming back

AUA defines recurrent UTI as 2 or more episodes in 6 months, or 3 or more in 12 months. If that is you, the question is no longer "which antibiotic" — it is why the same thing keeps happening: reinfection, a persistent reservoir, a stone, retained urine, or a resistant organism that empiric therapy never covered. Go to the ER, not to us, if: you have fever, chills, flank or back pain, nausea and vomiting (possible kidney infection); low blood pressure, a racing pulse, confusion, or dropping urine output (possible urosepsis); a known stone or hydronephrosis with infection (antibiotics cannot substitute for drainage); or you are pregnant.

Why was my urine culture negative?
Culture can miss fastidious organisms and polymicrobial infections, and prior antibiotics can suppress growth. A molecular panel looks for nucleic acid directly and covers more organisms.
How fast are results?
