Possibly because it was the wrong class entirely. Mycoplasma pneumoniae has no cell wall — and penicillins, amoxicillin and cephalosporins work by attacking cell-wall synthesis. Against Mycoplasma, they have no target. This is the mechanism behind the classic story: a lingering dry cough, a "walking pneumonia" that never quite resolves, and an antibiotic that made no difference.

If a cough has lasted weeks and a course of amoxicillin changed nothing, that is not treatment failure through resistance. It may be that the drug never had a target to begin with.
Go to an emergency department, not a lab, if you have: shortness of breath at rest, chest pain or pressure, blue lips, confusion, or a child breathing fast, grunting, or pulling in at the ribs.
