Molecular PCR the same day, from urine, a genital swab, or — critically — a throat or rectal swab if exposure was oral or anal. Pharyngeal gonorrhea is common and almost always silent. Treatment follows CDC, which narrowed its recommendation to ceftriaxone as resistance eliminated the alternatives.

CDC now recommends a single agent for uncomplicated infection, and monitors resistance nationally. This is why guessing at a leftover antibiotic is not just ineffective — it accelerates the problem for everyone.

Pharyngeal gonorrhea rarely causes a sore throat. It causes a negative urine test and a confident, incorrect sense of relief.

- Can gonorrhea be cured?
- Yes, with the currently recommended regimen. The concern is not curability today — it is the shrinking number of drugs that still work.
- Do I need a throat swab?
- If you had receptive oral sex, yes. It is the site most often skipped and most often positive.
- How soon can I be treated?
- The same evening. Result at 4:30 PM, partner-network physician, 4:30–6:00 PM.
- Will I know if I have it?
- Often not. Silent infection is common, particularly at extragenital sites.
